If the problem was that answered by Neus, then you should probably use formatted checkpoint file which you can generate from the checkpoint (.chk) of the calculation you already have (no need to rerun) by the formchk command. The .fchk (or .fch) file can be opened by all of commonly used molecular visualizers (e.g., Avogadro or Gabedit).

But to me, it appears that your transition is quite of mixed character. Wouldn't it be better to calculate and later visualize Natural Transition Orbitals? See Gaussian reference for their description.

For example, if Your excited state calculation was done with checkpoint file /home/igors/darba/IN18_438_TPSSh_6311_tolu.chk, and You are interested in the 3rd transition, you can run the following job:
